Camp Forms

These two forms must be completed for each camper.  If a camper is enrolled in multiple sessions, the forms do not need to be sent in again.  Please fill them out and send them to the Chevy Chase address indicated on the waiver form or bring them on the first day of camp.  Please do NOT mail them to the camp site.
